What changed
Zanzibar has concluded the three-year ZAN-TOTO Project, a 1.5 billion Tanzanian shilling effort led by the Zanzibar Health Research Institute with partners including WHO and ICARS. The project worked in five hospitals on antibiotic use among children under five, combining research with public education and awareness campaigns. Its coordinator said it reduced challenges affecting services by 25%, against an initial 30% target.
Why it matters
Antibiotics are not harmless all-purpose medicine. Used badly, they can prolong recovery, raise government healthcare costs and help bacteria become harder to treat. That makes this project’s focus on young children and their caregivers especially practical: better advice could mean fewer unnecessary treatments and more reliable use of medicines when they are genuinely needed.
The 25% result is encouraging, but its meaning remains limited because no baseline or measurement method was provided. It does not show whether prescribing improved consistently across the five hospitals, or whether antibiotic resistance itself changed.
The next test is whether the project survives its closing meeting. If Zanzibar’s Ministry of Health and the participating hospitals keep the guidance, training and medicine-management work in routine use, children and caregivers could benefit from more appropriate decisions. If follow-up funding and institutional ownership fade, the campaign’s gains may fade with them.
